The group is dedicated to promoting patient empowerment and participation through innovative self-care and patient centered approaches, to transform healthcare delivery to be more responsive to individual patient needs and preferences, to translate the evidence produced to polities, and ultimately improving health outcomes and quality of life. The group contributes to the WHO Collaborating Centre (e.G. Participation on WHO Curriculum Guide for Patient Safety participation). The scope of activities of this group will encompass several key areas:
- Developing behavioral interventions and self-management strategies to empower patients in managing chronic conditions, promoting healthy behaviors, and enhancing quality of life.
- Enhance patient engagement and shared decision-making in healthcare by developing tools, technologies and interventions that prioritize patient preferences, values, and needs and can lead to a better patient experience.
- Leveraging digital health and health technology, including mobile apps, wearable devices, and telehealth platforms, the group will develop and evaluate tools to support self-care and well-being, and enable remote monitoring and management of health conditions
.- Improving health literacy and education ensuring that patients have access to accurate and understandable health information to make informed decisions about their care, but also to navigate in the health system.
- Optimizing healthcare coordination, integration and continuity across healthcare settings, including transitions between primary care, hospital care, and social and community-based services. Translate research findings into actionable strategies and policies to promote patient-centered care and self-care practices at local, national, and international levels.
- Enhance cultural competence and diversity to create favourable contexts that impacts in self-care practices and patient-centered care outcomes delivery, with a focus on promoting equity and inclusivity.
